Your and Your Orthodontist brochure coverThe California Association of Orthodontist popular informed consent booklet for the orthodontic patient You and Your Orthodontist was revised in 2004.  Now Available to order.

This version was re-written and reviewed by orthodontists and legal counsel, the brochure is the most up-to-date and thorough.

Much of the language in the booklet is more "patient friendly" and easier to read and understand.

Originally developed in 1983, You and Your Orthodontist is a complete information and patient consent booklet, used by orthodontists across the country and around the world.

Over 300,000 booklets are ordered each year. Most likely, the booklet is used in your community by orthodontists you know.

Improvements from Previous Version

A task force of orthodontists carefully reviewed the prior version of You and Your Orthodontist, last edited in 1998. They both changed and added material to the booklet.

In fact, the new version includes 11 new sections that bring this important patient education and compliance tool up to date.  There are significant changes to the consent form. These include statements about question and answer opportunities, the inability to guarantee results, and a witness signature line.  The tear out Consent Form(p15) is designed to match the patient copy of the Consent Form(p16), so that orthodontic offices can put a piece of carbon paper in-between p15 and p16, so both the doctor and patient will have a signed copy.

All are designed to give greater protection to both the orthodontist and patient.
